Dear parents,
The Instituto Thomas Jefferson continues with a healthy plan, offering to you the Influenza Vaccine 2011-2012.
The only contraindication to implement this vaccine is egg allergy, and / or chicken protein.
The vaccine price is $300 pesos.
If you are interested on getting this vaccine please complete the attached format. It is important for you to write down the complete name and grade of your children and the people who is getting the vaccine.
Please deliver the format with the names and the money in an envelope directly at the nursery. The vaccine payment will be only received from September 27 to October 3.
The vaccine application will be from October 4th to the 14th, from 9:00 am to 3:00 pm.
